I've had both, depends which doctor/nurse I see. I think the automatic ones are coming into vogue though.
If the automatic cuff hurts so much it's driving your BP up, then say so while you've got the cuff on! "OW OW OW it's hurting me please take it off!" If possible, say so beforehand - look, this cuff always hurts my arm so much that it actually affects my BP reading; please use a manual cuff or else I must refuse to allow you to take my BP today. (In the US, it is your legal right as a patient to refuse any medical treatment or procedure that you wish).
