Just fished mine out and it is 5 years but the accompanying letter says they'll send a reminder when it's due for renewal.

no idea where mine is or when it runs out, and moved house at least 3-4 times in the last 5 years without letting them know so I won't get their reminder letter. Have been diagnosed 8-9 years now so hopefully it's not due to run out for a while yet.

Looking at the letter that comes with, the phone number listed is 0300 3301641, says there's a website, www.nhsbsa.nhs.uk/healthcosts It says you can phone or email with a change of address but I don't know if you can check whether yours is in date without drawing attention to it.

Basically if it is out of date you do need to renew it, so drawing attention to the fact shouldn't be a problem.  Whist renewing you need to get a proper receipt for any prescriptions from the Pharmacy (NOT a till receipt - ask, they know what you need) and then claim back any charges.
